This very much depends on your router model and how you have it set. If your handset is in use (offhook) and a call comes through, normally the router will send the caller a busy tone. If you have a router with two voice ports (e.g. Vigor2600V), the router can optionally deliver the call to the other VOIP port. To enable this, from the telnet interface enter voip sip epfwd 1 and use voip sip epfwd 0 to disable it.
If your router supports 'call waiting' you will hear a beep indicating a waiting caller and you can switch to the new caller by pressing 'Recall' (TBR) on your telephone (not 'Redial').
